Аннотации:
Проведено ретроспективное исследование по изучению отдаленных результатов
при лечении сарком мягких тканей с применением интраоперационной лучевой терапии на малогабаритном бетатроне БМ-10Э (n = 171) в сравнении с группой комбинированного лечения с адъювантной ДГТ (n = 57) и группой хирургического лечения (n = 171). Применение ИОЛТ как компонента комбинированной терапии достоверно улучшает показатели 5-летней общей (p = 0,025) и безрецидивной выживаемости (p < 0,025), а также
показатели 5-летней общей и безрецидивной выживаемости при размере опухоли более
5 см (р = 0,0001). При этом различия в показателях безрецидивной 5-летней выживаемости между группами в выборке умеренно- и низкодифференцированных сарком статистически достоверны (р = 0,00002), а различия между группами в показателях общей
5-летней выживаемости в зависимости от степени дифференцировки опухоли статистически недостоверны(p = 0,33), однако прослеживается отчетливая тенденция к уменьшению показателей выживаемости при умеренно и низкодифференцированных саркомах (G2-3), что позволяет позиционировать комбинированный подход с применением ИОЛТ
как альтернативный метод в лечении сарком мягких тканей умеренной и низкой степени
дифференцировки. We have performed the retrospective research upon the follow-up study in the treatment
of soft tissue sarcomas using intraoperative radiotherapyby means of the compact betatron
BM-10Э (n = 171) in comparison with the group of combined therapy of adjuvant gamma-ray
teletherapy (n = 57) and the group of surgical treatment. The using of intraoperative radiotherapy
as the element of combined therapy is confirmed to improve the indices of 5-years overall
(p = 0,025) and progression-free survival (p < 0,025) as well as in the indices of 5-years overall
and progression-free survival with the tumor size more than 5cm (p = 0,0001). In such
a case the differences in the indices of 5-years progression-free survival between groups in sampling
of moderate- and poorly differentiated sarcomas are statistically reliable (p = 0,00002)
and the differences between groups in the indices of 5-years overall survival depending on the grade of tumor differentiation are statistically not reliable (p = 0,33), however, we can observe clear tendency to decreasing of survival indices in moderate- and poorly differentiated sarcomas (G2-3), that’s why the combined approach with the usage of intraoperative radiotherapy is allowed to be considered as the alternative method in treatment of the soft tissue sarcomas of
moderate and poor differentiation grade.
